  2. Recent 2 x 2 inches size photo with white background (4 Copy Mat Paper)
  3. NID Copy (English Notarized)
  4. TAX Return Copies in English (Last 3 Years)
  5. TIN Certificate
  6. Marriage Certificate if Applicable (In English & Notarized)
  7. GD Copy if Any Lost Passport (In English & Notarized)
  8. Affidavit (If Any Name Changes in Previous Passport)
• For Job Holder
  1. N.O.C or G.O. Letter
  2. Office ID Card & Visiting Card
  3. Salary Certificate
  4. Personal/Salary Bank Statement & Solvency Certificate (Last 3 Months)
• For Businessman
  1. Trade License Notarized Copy with Translate in English  
  2. Personal & Company Bank Statement, Solvency Certificate (Last 3 Months)
  3. Visiting Card
  4. Company Blank Pad (3/4 Copies)
  5. Membership, Memorandum, Incorporation, Export, Import Certificate (If available) (Notarized)
